Donation Request Form

All requests need to be made via this online form. Requests made in person, email, mail or phone call will be directed to fill out this form.

We are currently accepting requests for up to 4 rounds of golf for the 2025 season from one of the Total Golf Management family of golf courses (Canterbury Woods Country Club or Pembroke Pines Country Club located in NH). Only one course can be chosen for the request. Alternatively, you can request a $25 gift certificate to one of our restaurants (Canterbury Tavern or 1759 Grille) instead of the rounds of golf.

All donation requests will be reviewed within two weeks. The organization requesting the donation will be contacted by email, regardless of whether or not the request is approved.
